If you came looking for a wedding inquiry form template or a wedding contact form to download, Hailo does not offer a file. It gives you a form builder instead, and every new form starts with the questions most wedding filmmakers ask, so you could publish in a minute and edit from there:

• Your name and your partner's name

• Wedding date, with a hint to leave it blank if it is not set yet
• Venue or city
• Approximate budget, as a dropdown
• How did you find us, as a dropdown
• Tell us about your day, as a paragraph

Questions you write, in a look that matches your brand

Click any label to rewrite it, drag rows to reorder them, and add questions as short answer, paragraph, email, phone, date, number, dropdown, multiple choice or checkboxes. Any question can be required. Name and Email are locked in, so every inquiry comes with a way back to the couple.

Then pick a look. Match my brand reads the logo, fonts and colors from your brand kit, while Editorial Cream, Cinematic Dark, Letter and Magazine Spread carry their own palette and type. On your own brand you set the accent, the button, the background (transparent works, so your site shows through) and the field fill, then choose one or two columns and how round the corners are. Your brand kit itself lives under studio setup.

The form on screen is the editor, so the headline, the intro line and the submit button are all edited in place.

On your website, or anywhere you can paste a link

Copy one line of HTML and paste it onto your site. It works on Squarespace, WordPress, Wix and custom sites, and it resizes itself to fit, so there is no scrollbar inside your page. Where you have no website to paste into, copy the form's direct link instead: your Instagram bio, a Linktree, a QR code on a printed card.

After a couple submits, show a thank-you message where the form was, or send them to a page of your own, such as a pricing PDF. You can run several forms, each counting the leads it captured, and pause one without deleting it. That makes a website inquiry form for the main site and a second one for a landing page easy to tell apart.

What happens when a couple hits send

The inquiry lands on your Leads page as a new lead with the source Website. Names, email, phone, date, budget, venue and message fill the lead's own fields, and every other answer is kept under Form responses on their page. Hailo emails you and rings the bell, and any automation you built for new leads starts. What you do from there is covered under client records.

Turn on the auto-reply and the couple hears from you right away, from your own connected mailbox, in your words. Merge fields fill in their first name, and Send myself a test shows exactly what they will receive before it goes live. If you would rather send the reply and a discovery call invite two days later as one sequence, a starter automation does that instead.
