Questionnaires

Build a questionnaire in four steps, send it from a client, a booking or an automation, and see what the couple fills in.

A questionnaire is a form you send a couple to collect wedding-day details: ceremony time, venue addresses, the must-have people, music dos and don'ts. Build it once under Templates > Questionnaires, then send it from a client, attach it to a booking, or let an automation send it on schedule.

The list

Columns are Name, Questions (a count) and Description (your internal note). Click a name to edit. The subtitle reads "Forms you send to clients to capture wedding-day details, vendor lists, music preferences, anything you want them to fill out." With none saved, the page offers New questionnaire.

Build a questionnaire

Click New questionnaire. The builder is four steps down a rail on the left: Details, Questions, Welcome & thanks, Review & send. On a new questionnaire steps 2 to 4 unlock after you save step 1.

Step 1: Details

"What are you collecting? Give it a name you'll recognize later. The couple never sees the internal note."

  • Questionnaire name, for example "Wedding-Day Details". The couple sees this as the form's title.
  • Internal note (only you see this): when to send it, what it covers.

Click the continue button to save and move on.

Step 2: Questions

"Build your questions. Add each thing you want the couple to answer."

  1. Type the question in the box ("e.g. What time does your ceremony start?").
  2. Pick a Type.
  3. For Multiple choice and Checkbox, a Choices list appears. Type each choice; click Add choice for more and the × to remove one.
  4. Click More to reveal Required and Help text (optional, shown under the field).
  5. Click Add.

Each saved question appears as a row with its number, label and type. Use and to reorder, × to delete (confirm "Delete this question?"), and click the row to open its edit panel: Question label, Type, Required, Choices, a live Preview (what the couple sees) and Help text. Click Save; the button flashes "Saved".

Note

Older questionnaires may contain questions of type date, time, number, address or file upload. Those types are no longer offered when adding a question because couples answer them fine as text, and file upload never stored files. Existing questions of those types keep working, and the edit panel keeps the type as "(current)" so a save does not change it. If you have a file-upload question, replace it with a short text question that asks for a link.

Step 3: Welcome & thanks

"Wrap it with your voice." Both fields support Markdown.

  • Intro (shown at the top of the form): "Welcome the couple, set expectations, tell them how long it'll take."
  • Thank-you message (shown after they submit). Defaults to "Thanks, we have what we need!"

Step 4: Review & send

"Ready to go." A summary shows Name, Questions and whether an Intro is set. Click Preview as the couple to open the form as they will see it (the submit button is disabled in preview). Click Done to return to the list.

To delete a questionnaire, click Delete in the builder header. The confirmation reads "Delete this template? Existing client sends keep working." Couples who already have a link keep it.

Send a questionnaire

From a client

  1. Open the client and click the Forms tab.
  2. Under Send a questionnaire, pick the template and click Send.

Hailo creates a unique link for that couple and emails it from your connected mailbox: "When you get a chance, fill this out so we can tailor your film to you". The send is listed in the tab with a Public link and its status (sent, opened, completed). When the couple submits, the answers are shown there.

Message What to do
"Questionnaire created, no email on file for . Share the link: …" Add an email to the client, or copy the link and send it yourself.
"Questionnaire created. Connect an email account in Settings → Integrations to send it automatically, for now, share: …" Connect Gmail or a custom mailbox. The link already works.
Note

Questionnaire sends belong to a client. A lead that has not been converted yet does not have a Forms tab; attach the questionnaire to the booking instead, or wait until they book.

On a booking

In a lead's Booking tab, step 04 Intake questionnaire: turn on "Ask this couple a few questions" and pick the questionnaire. The chip changes from Not picked to Attached. The questions become a Questions step on the couple's booking page, headed "So we can plan the day" with the note "These help us hit the ground running, most couples breeze through in 3 minutes." The wording is frozen when you publish, so later edits to the template do not change what that couple is asked. See Booking page.

From an automation

Add a Send questionnaire block ("Drop a questionnaire link in their inbox.") to an automation and pick the form. The "Pre-wedding questionnaires" preset sends three of them at 90, 60 and 30 days before the wedding. See Build an automation.

What the couple sees

The form opens in their browser with the eyebrow "Questionnaire · prepared for you", your questionnaire's name as the title, your intro, then each question with a red asterisk on required ones and your help text underneath. Colours and fonts follow your brand kit when your theme is set to custom. They click Submit and see your thank-you message on a card. Opening the link marks the send as opened; submitting marks it completed and stores every answer, including every ticked box on a checkbox question.
